This is something I saw with NetBSD 1.6.1 on my MTRON SSD w/ACARD SCSI to IDE adapter.  Recently 5.0 was made to work again on Atari and so I've now tested there with the same results.

This issue doesn't happen immediately, but once it happens it seems to take out the system pretty quickly.  That kind of makes sense, being that it affects disk operations.

The following screenshot shows the problem during an install of 5.0 RC2:

There are no other devices on my SCSI bus, just the MTRON/ACARD combo.

Attempt to install/use NetBSD 5.0 RC2 on an ACARD SCSI to IDE bridge with an MTRON IDE SSD attached.  More info on the exact hardware specs can be provided upon request.

After a while, you'll see various messages from the kernel reporting "Check Condition on CDB", some #s, "SENSE KEY:  Illegal Request" and "ASC/ASCQ:  Invalid Command Operation Code"

I do not experience this problem with a standalone Seagate SCSI drive.  I have no reason to believe that the MTRON/ACARD hardware is faulty in any way, I haven't experienced any problems using it with a PC and Adaptec SCSI controller (though not with NetBSD).
