 Could you elaborate on what the problem is?

 First of all, I assume we are talking about the file at
 http://www.netbsd.org/docs/guide/download/netbsd-en.pdf.

 When I look at the file (in Atril), it displays normally on the page.
 In the index, there are separate entries "Chapter 6" and "Editing",
 instead of a single "Chapter 6: Editing". Is that what you mean?
