The internal speaker is not muted when using headphones on 945GMS Express chipset machine (Fujitsu FMVLP70WN).

This report includes dmesg with AZALIA_DEBUG, output of mixerctl -av, and my /etc/mixerctl.conf.
I think outputs.unknown0b as the internal speaker and outputs.black0a as the headphones in output of mixerctl.

 With the new hdaudio, the internal speaker don't work (no sound),
 but I can use headphones without use of mixerctl.

 I attach dmesg with HDAUDIO_DEBUG and HDAUDIO_AFG_DEBUG options, and output of 'mixerctl -av'.
 I have no /etc/mixerctl.conf now.
